WAIVER AND RELEASE OF LIABILITY
2026 West High Alumni Hockey Tournament
Dates: April 29, 2026 – May 2, 2026

1. ASSUMPTION OF RISK

I acknowledge that participation in ice hockey and related activities involves inherent risks, including but not limited to:

  • bodily injury

  • fractures and sprains

  • head injuries, including concussion

  • collisions with other players, boards, or equipment

  • equipment failure

  • ice surface hazards

I voluntarily assume all risks associated with participation in the 2026 West High Alumni Hockey Tournament.

 2. RELEASE OF LIABILITY

In consideration for being allowed to participate, I hereby release, waive, and discharge:

West High Alumni Hockey Tournament organizers, event staff and volunteers, rink owners, and operators, sponsors and affiliates from any and all liability for injury, illness, death, or property damage arising from my participation, whether caused by negligence or otherwise, to the fullest extent permitted by law.

 3. MEDICAL AUTHORIZATION

I certify that I am physically able to participate in this event. I authorize tournament organizers to obtain emergency medical treatment on my behalf if necessary. I agree to be financially responsible for any medical care provided.

 4. EQUIPMENT AND CONDUCT

I agree to:

  • wear appropriate hockey equipment

  • follow all rules and regulations

  • conduct myself in a safe and sportsmanlike manner

I understand that failure to comply may result in removal from the tournament without refund or the ability to attend future events and tournaments.

 5. PHOTO AND MEDIA RELEASE

I grant permission for my image, likeness, and voice to be used in photographs, video recordings, and promotional materials without compensation or further approval.

 6. NO REFUNDS

I understand that registration fees are non-refundable unless otherwise determined by tournament organizers.

 7. ACKNOWLEDGMENT

I have read this waiver and fully understand its terms. I understand that I am giving up substantial rights, including the right to sue, and I sign it freely and voluntarily.
